How do I opt in to Workplace Pension

Hi all, so my new employer sent me a letter telling me I could opt in but is there a template letter to do so? I have googled but all I keep finding is the employer template letter not an employee one. On my letter it just says to contact boss in writing by sending signed letter or send it electronically containing the phrase "I confirm I personally submitted this notice to join a workplace pension scheme."

Can anyone please help me word the letter? Also I work alone in an office and will be emailing so will I need to write the letter then scan and attach with the phrase above or do I just write it all in an email? Boss on hols so can't ask them and friends I have asked are just as confused.
