Claiming tax relief

I managed to get a small pot using the small pots rule before change of year ie 2018/2019.
They as expected took tax off which I will need to reclaim.
Can I ask a few questions about this.
When claiming – is it the P53 form?
If so - Most of the stuff on this form is fine - but there are a few things I was not sure of :
P45
At the end they say to attach parts 2 and 3 of your original form P45. They say your pension provider should have given you this form – I don’t have this and as yet have not received anything from my SIPP provider.
Thanks any help with this would be great.
