Will I get a bad reference and will it affect my application?

You cant just pick your friends as references! Plus it’s at the same workplace, they’ll know if you’ve picked Betty the colleague over Frank the ACTUAL mananger.

Regardless of how many jobs the OP has had in between they will of course go to the manager from when they worked there, even if not listed as a reference. Omitting it looks even more suss.

I worked in Italy for over 7 years and I found it amazing that a member of staff responsible for basically cleaning the garden listed himself as a concierge who liaised with clients and managed staff shifts on Linkedin and is now a (without a reference) an employee of Booking.com To make it even worse my actual concierge worked her !!! off while he would only work to his contracted hours.

TBH its 2019, get whatever references you can, it doesn't really have to be your last manager because lets face it, people know even managers can be crazy pieces of !!!! and their opinions may not even matter.

Saying that i can imagine working in a public health position is a lot more difficult....even though healthcare managers are also as !!!!ing useless

Given public health and healthcare are totally different i'm not surprised by your opinion...
