Prize to be collected from local opticians, select from list of nearest 3 which appear when you put in your postcode.
I needed to input Name & Address details by typing in to make the postcode work correctly and bring up the Optician options. The Optician selected then appears in a further box. If you don't do it right, the additional box doesn't get populated and your entry won't go through.
NOTE: You need to scroll through to the end of the T&C's box to activate the T&C Agreement box.
