When did you work for this employer - your payslips will have dates on them.0

Why not ask a separate question of any colleagues you may have had at the company during the time you worked ther and see if they have more details. I know its a long time ago, but social media may be worth a try. If you were there for a few years in your middle age chances are people you knewand were friendly with may be reasonably settled and easy to track down - and I imagine would be willing to provide basic information such as the name of the scheme and maybe even a bit more
linkedin may be good as people tend to put heir work history on it - so just search for your company name - even if smallI think I saw you in an ice cream parlour
