Tax computation & overview

Hi everybody. Me and my girlfriend are applying for our first mortgage and we've been asked to provide some additional information to our mortgage broker as I am self employed. This is a tax computation and tax year overview. As I complete and submit my own self assessment forms (everything declared) I'm not sure how I get this. Can anyone help?
