Hello,
I've been off from work for 4 weeks. I hoped I would be off for less than a week so I thought I would be fine with a self assessment. Unfortunately I got worse and in the second week tried to see a doctor for a fit note. Due to having to change surgery as I'd moved and horrendous waiting times it took me 3 weeks to see a doctor.
The doctor has signed me off for a further month but can't back date it to cover the missing 3 weeks. Does my employer have to pay me SSP for the 3 weeks without a fit note or is it at their discretion?

No they don't have to pay. You didn't provide a fit note. The reason why you didn't is irrelevant.0

What did the GP say about back dating the sick note?

If a GP knows the patient they may be willing to backdate a med cert if the surgery can't provide an appointment any earlier. That will only apply to existing patients. The OP has changed surgery so the GP won't, or certainly shouldn't, provide a backdated certificate. Too late now but the OP should have said they needed an urgent appointment.0 -

Well an urgent appointment should be based on medical needs, not paperwork ones!0 -
