Mortgage in principle processed twice by same lender, first accepted, second refused

Having a melt down! Applied for Decision in principle with Nationwide a week ago, it was accepted. Since had offer accepted on a flat and went to continue on to apply for
Mortgage online. The online form was playing up and wouldn’t let me add address information for the property I was purchasing. I called helpline who said I had to go back and it wa earlier in the process. I looked at all the links on my mortgage application homepage and found the address section BUT it’s submitted a SECOND decision in principle and NOT the mortgage application and it WAS REFUSED. My original decision in principle still sits in the document list but above it I have another one saying declined.
I’m going to call Nationwide tomorrow and explain it was not a Dip and see if this can be wiped. anyone has similar issues?

you need a broker to be honest as that would make the process more streamline and you get access to lenders who are suitable individually
I avoided all that filling in with my broker, just a quick phone call to them and a quick expenses and income spreadsheet and they do all the work"It is prudent when shopping for something important, not to limit yourself to Pound land/Estate Agents"
