Transfer of ownership, equity & a charging order :(

I have an issue which I could do with a bit of help with…
I co-own a house both in deeds and in mortgage with my now ex wife
I want to take sole ownership and affordability is not an issue
My credit rating is poor but it should be okay (words from Yorkshire Building Society)
There is a charging order on the house due to unpaid debt, subsequent CCJ in my ex wife’s name.
I’m not certain whether this is just a restriction or a full charge order.
It’s around £2500 of which she has just started to pay back around £150 per month.
How do I go about the transfer of equity under these circumstances, and is it possible?
No money will change hands so there’s no issue there either.

Any advice would be great.
