Remortgage advice needed

Hi.
I have a mortgage with Nationwide of which the 5 year fixed rate is due to finish at the end of December 2018. During the last 4 years I have topped up my mortgage 3 times. It wasn't until the last time i did this in 2017 that I found out I was in fact taking out new mortgages each time - my stupidity thought i was adding the amounts onto my existing mortgage and i thought the fixed rate for all would end at the same time this year. Except now my main mortgage fixed rate expires in December 2018, then the next one expires Oct 2019, then February and August 2020 ! I do not feel comfortable with this at all and i'm wondering if I can remortgage all of them to combine into one mortgage? I presume it would be best if I wait until December 2018 to do this (???) as the early redemption fee on my main mortgage is almost £4,000.
I am really useless at this kind of thing and so any advice at all would be very much appreciated!
