I did not have the option to enter my details when I answered the questions with the 'no sleep problems' options, but when I went through it (in another browser!) and said I had sleep problems, then the personal details boxes came up. They are evidently not interested in anyone who sleeps ok.
  • I completed the survey and I do have sleep problems...but it didn't ask for any personal details.
  • Does anyone know the source of the survey? Thanks.
  • I completed it WITH sleep problems and was asked for my name and email.
