Back for more help soooo ashamed!!

Hi guys got some good advice last xmas and it stood me in good stead!!! but unfortunately I am in a rather sticky pickle again!! Let me explain
Thanks to this forum i joined Payplan...and was paying them £110 per month and all was going ok i was getting by,,,My wife is back to work this last 5 months and we were just about getting along. We had 2 chilren in succession and for the 2nd my wife fell down the stairs and broke her foot whilst pregnant so had to go on Maternity leave early ..then just before the baby was born her brother was viciously assaulted and was in a coma for a month so Jolene suffered from severe depression and it took a while for her to get back to work... Meanwhile not wanting her to worry me being the sole earner was taking its toll and i was not gonna stop her driving to the hospital every day blah blah you get the picture.. I ended up robbing peter to pay paul as the old saying goes....so as I said wifey back to work now past 6 months but still catching up on year of rent, oil and car arrears etc...Now have received a Rates bill which they say they want £320 per month and that has pushed me to the edge to contemplate leaving my wife and kids as we would be financially better off not living under the one roof.....I am at my wits end as christmas is coming and I do not want to let my kids down
here is my S.O.A
My INCOME..£1224 Per month
Jolene £752 Per month (£188 per week home)
Child Tax Credit £ 450

Other JOb £240 Per MOnth

Total.... £2786
Outgoing
Rent £525
HP Car £193
Creche £900
Credit Union £114
Payplan £110
Electricity £63
Diesel £140

Oil £45

Total £2508
so then
£2706 minus £2508
leaves 198 per month....yet I have at this minute £180 in bank....
That needs £40 for Diesel £30 for childrens nappies wipes etc...
childs present for xmas @£40 pounds....

Why then am i in arrears of 250 to the oil man The rent payment is Including £100 of arrears each month... am in arrears in the hp of car by about £150...
Help i should not be in this position... is there a way to re-arrange my finances as we are both earning now i do not want to end up homeless
