Self certification form for SSP.

How do I get hold of a self certification form for sick pay?. I don't qualify for a fit note as I am going back next Monday and have only taken 3 days by then.

  • I think your company is likely to have its own - companies I have worked for certainly have. Ask HR maybe?

    I have been asked to get one and send it to payroll. Can I go to my doctors or any surgery and obtain one?.

    You used to be able to download from HMRC.

    https://www.gov.uk/government/publications/statutory-sick-pay-employees-statement-of-sickness-sc2 - but only relevant if you've been ill for 4+ days. Other info here

    If you have only been off sick for 3 days there is no entitlement to SSP.

    It is worth remembering that to be counted for SSP purposes you need to be off sick for 4 consecutive days. These are any days not just days that you work.
    So if you normally work Monday to Friday and were sick Wednesday, Thursday and Friday, and returned to work on Monday it could make a difference which day you declared as your last sick day. If you declared your last day of sickness as Friday you have three days of sickness so this is ignored for SSP, but if you declare Sunday as last day of sickness you now have five days of sickness so this is recorded for SSP.
    This will make a difference if you have been off sick in the last eight weeks or if you go off sick in the next eight weeks. If this happens any periods of sickness will then be treated as one single period of sickness with only one period of waiting days with no SSP payment.
    For example; off sick in last eight weeks and you were getting SSP then you would get SSP for the Wednesday, Thursday and Friday; off sick in next eight weeks for at least four days then you have already served your waiting days and SSP is payable from the first qualifying day.

    No. It's nothing to do with your doctor. They issue fit notes after you've seen or spoken to them about your illness. This is a form which you fill out yourself. If your company doesn't have its own form, then google for a template and download it and fill it in.
