Baffled by tax certificate on dividends

I am completing a tax return for 16/17 and struggling with box 5, the 'other dividends' section that includes Unit Trusts, OEICs etc. My wife and I have a joint investment of such funds through Aegon and I have found the tax summary statement. The 'total dividend distributions' was £380, which (my half share of) would seem to be the amount to declare. However also on the statement is a certificate of deduction of tax, which lists 'Rebates' at £69, tax deducted £13, net £55.
What is this Rebate and where (if anywhere) would it fit into the tax return? My income for the tax year will be less than 10k so if I have paid even £13 tax unnecessarily I would aim to reclaim it.
