P11d self assessment advice

Hi,

I'm filling in an online self assessment tax form for the very first ever (yay!)

However I am confused. I receive medical benefits from my company, they are listed in my P11d document:

The part of the p11d is section "I" and my benefits are listed thus (where x is the figure on the document)

Private medical treatment or insurance

Cost to you : £x
Amount made good or from which tax deducted : £0
Cash equivalent : £x (same figure as "Cost to you")

My question is; what do I do with this figure when filling in my tax self assessment? Is there a part of the form where I need to declare this, and if so which page is it on?
