I think you are in the right here and should stick to your guns.
Purchase Order does form part of the contract with the buyer, the bottom line here is they didn't pay! If the person who made the booking wasn't authorised to send a PO, then that's not your problem, it's the MD's. I think there's a bit of brinkmanship here: he's just trying to get out of paying for it. Which is a difficult place for the charity, not you.
Did you follow the same process in handling the booking as you would have done with any other booking? If so, it's the process that's at fault, not you.
What you have done in following up for payment doesn't constitute harassment.
From what you've said, I don't see any grounds for a disciplinary meeting. It might be an uncomfortable conversation though but I would go with clear list of events (i.e. the facts of what happened written down and copies of what you have sent) to discuss. Be objective and focus on the charity's best interests. They are in an awkward place and have to balance reputation with income, but that is what they get paid for. If you feel you are getting upset and can't be objective, don't be frightened to ask to have a break from the discussion until you feel better. You could ask in advance what they would like you to bring to prepare for the meeting. I would also ask your managers what you should have done differently and what the charity's policy is for following up debtors.Debt 1/1/17 - Credit Cards £17,280.23; overdrafts £3,777.24

I'd say its the Op's job. If I was a business owner I'd be pretty annoyed if the new boy/girl thought customers didn't need to pay!!!!0 -
well one of our customers didn't pay us when they said they would & having been given the run around for a month I lost my rag with them.
At no point was I rude but they were left in no doubt that I wanted payment sooner rather than later ....in fact after my conversation, my boss asked me to remind him never to get on the wrong side of me lol.
It's a pity the OP hasn't updated us but hey ho!0
