EIS Enterprise Investment Scheme Tax Relief Claim Form

Hello

was wondering if someone could help me please.

I invested some money in crowdcube funding website. And the company became eligible for the Enterprise Investment Scheme.

Which allows income tax relief for money invested in start ups by people.

I have received the certificates from the HMRC Tax man to the company.

I need to fill it in, but I'm not sure how to do it.

It looks like a very simple 2 sided page form that somehow will allow me to claim back income tax i think on the amount i invested. But I'm so unfamiliar with the terminology, I have no clue how to fill it in.

Could someone advise? Or should i go to an accountant?

I really don't want to spend £500 paying someone to fill in just a 10min form by the looks of it.

this is the 2 page form, i think I'm only meant to fill in both sides of the 2nd page and send it off to hmrc. But not sure with what EXACT details.....?

P.S. if it makes anything easier, i don't have a mortgage, assets, loans or investments of any other kind. I just invested in shares in just this one company. for me it was a significant amount of money, probably nothing to most people. Apart from this investment i have nothing else.

I work, pay taxes and rent, single, unmarried no kids. No benefits, tax breaks or anything. Probably am eligible for some of them as i work in uniform job role in customer service.

With overtime i have hit the 40% tax threshold a bit last year or two. But most part of my income falls into the 20% lower income tax threshold.
