    Has anyone else had trouble submitting the form? I have tried in 3 different browsers, and keep getting the message 'all fields must be completed' (or similar).

    I have tried tabbing in case there is a hidden field but nothing. Have tried both roboform and manual submission.

    Any tips would be gratefully received :D

    Continually had same problem as above and on the 6th or 7th time pressed for a capture reset before completing and submitting and it worked... not sure if that is of any relevance but hopefully may work for you :o
