A transfer is the legal process to remove someone from the ownership of a property when the mortgage has been resolved to the one remaining party.

You need to seek legal advice on the claim before any transfer of equity can be completed.

There's your problem. Suggest you speak to a solicitor to formalise the financial settlement. While you've paid the mortgage, she's had housing costs of her own to bare.
