Tax office says I haven't been working for the past 3 years?

Hi!
So I've been working part time for the same employer since 2014, on a zero hour contract earning on average 112 a week.
I've been on maternity since the end of November 2016.
I was due to be going back on the 18th of next month but they have told me due to me not wanting to go back working days but evenings instead they don't need me- which is fine.
I managed to get another job, I rang up the the tax office to change this as when I started this second job I was still planning on going back to my 1st job so on the starter checklist I ticked this is my second job.
After speaking to them they have said that it's not on their system that I've been working for the past 3 years.
1st question; Will I still be entitled to a p45 from my old employer? As I kept asking Them for my p60 but nothing has come of it.
2nd question: am I still entitled to holiday pay from my old employer while being one SMP?
3rd question: I assumed I had only been paid 38 weeks SMP, after speaking to my employer it turns out it's because my first SMP payment was £0 why would this be?
Anyone who could give me an answer to any of these would be much appreciated thank you.

My immediate thought is that a wrong NI number has been used by the employer. The OP needs to check that with the employer as a first step.0

OP, do you have your wage slips? these would usually show your NI number and what Tax and NI you have been paying, and may be helpful in checking whether they have your details wrong.
Your employer should provide you with a P45. Have you requested this (and your P60) in writing?

You say you've been earning on average £112 per week. That's not enough to pay tax, and is on the cusp of the threshold for starting to pay NI (£111 in 2014-5, £112 in 2015-6 and 2016-7, and currently £113), so if you've actually earned below the threshold the whole time then there would have been nothing for you to hand over to HMRC, so they would have no payment record for you.0

I have a few wage slips not many though!
If this is the case about them not having to report me to Hmrc for working, how do I go around ensuring my SMP has been paid correctly regards to my first payment being 0 as my employer hasn't a clue and when I rang Hmrc they told me they can't look into it as they've not been given any information about it.
I worked over 20 hours most weeks but would they only put 18 through on my wage slips ��
I have got a number recently to report them if I do not receive my p60/p45 so I will give them another week first!0
