Doesn't seem to be submitting in Chrome?0

In Chrome the only way I could get it to submit was going through an incognito link.
Right click on the link and choose incognito from the drop down menu. You can fill in the form but will have to confirm your phone number - they send you a number to put in.
It should then submit
