what sick form do i need

ive just been in hospital for a kidney stone operation, I asked for a self cert there but they said I was to get one from my gp, however I downloaded one and handed it in, my people manager has called to say its the wrong one, I handed in the one for ssp when I will get company sick pay, however I cant find this form online, will the gp one be different?

    You need to ask your employer.

    A standard "Fit note" (what used to be called a sick note) is, strictly speaking, for SSP purposes only. You need one if you are off for over a week.

    If you get company sick pay over and above then the firm's own rules apply. They might, for example, want a doctor's certificate from day one of your absence and the doctor would generally charge a fee for that as it is not part of their NHS duty.

    how long were you off for?

    Neither the hospital nor the GP will issue a 'self cert'. As the name suggests, this is self certification which is generally allowed for the first 7 days of a period of absence.
    If you employer wants a certificate beyond that period it will normally be provided by the GP.

    The form to self certify is usually got from the employer when you return to work.

    I think you are confusing a medical certificate (now referred to as a fit note) with self certifying the first 7 days of sickness. You can't complete a medical certificate yourself as it needs both a doctor's signature and a surgery stamp.

    There used to be a self certification form that you got from the receptionist at the GPs for the first 7 days but this was scrapped. Now if the employer wants a form in the first 7 days they provide their own unless it's for SSP which is the form you found.
