I apologise: I hadn't noticed that your wife had health problems too. Perhaps you'd not attempt to top up her state pension until shortly before it is due, if at all.

So I revert to the minor idea of personal pension contributions for your wife, and the major idea of getting quotations for ill-health annuities.
