Gross Salary, Child Care Vouchers & Mortgage Application

Just a couple of questions re gross salary.

My husband and I both claim £243 a month pre tax Childcare Vouchers, this more than covers our childcare costs and will continue to do so later in the year when our eldest starts school age our youngest starts nursery. Neither of the P60s we've been issued this month mention CCVs and so our gross salaries, of course, demonstrate our post CCV income. My husband's p60 does not show his pension deductions either, but this has clearly also been deducted from his gross wage.
Both our payslips DO show both pension and CCV deductions.

When completing an application form online, do I give the income as shown in the p60 and say our childcare costs are nil? Or, calculate our pre deductions salary, based on the payslips, and then confirm our childcare costs of just over £400 a month? Is with First Direct, if that makes a difference.

In addition, do you know how far ahead you can lock in a deal with First Direct? Hoping to fix it for October so that my current maternity leave does not become an affordability issue for them. Thanks!
