Re-mortgage whilst on debt management plan

Good afternoon and thanks fir taking the time to read this.
I was wondering if there were many companies with products for someone wanting to remortgage whilst on a debt management plan. ?
The ghosts of my unfortunate past seem to be firmly gripping my ankles and stopping me moving forward toward my debt free goal. I have now secured a position that is infinitely more secure than my previous trade and also pays a lot more , so I would like to consolidate my remaining debt of around 12000 and release some funding to convert my loft at the same time.
Any help and guidance would be greatly appreciated.
