Self assessment tax return help

1_Just checking that the self assessment form for january 2017 covers the period april2015to april 2016?
2_Ive tried to log in online but I cannot remember or find my details. Do I just call them up tomorrow?
3_ive done a mix of paye and freelance . I imagine it will be under 10k. Anything I should bear in mind?
4_I believe im late on nic due to illness and being broke for past outstanding money so far (as got other debts too trying to sort out). Will this cause any issue?

    2. You've left this far too late. IIRC they will send out new details by post. Why are you only doing this now? It's unlikely to get to you by the deadline. Have you tried to see if you can recover your details online? It's your only hope of avoiding a £100 fine.
    3. If your total income is below the personal allowance you will have no tax to pay. If your self employed earnings are below the lower earnings limit you will have no class 2 NIC to pay either.

    in this day and age of technology i thought getting access to inland revenue with ones details would be automatic like it is for any bank or most financial things nowadays. seems to make sense? why wait a week?

    You have lost all your login details. There is no way you would be able to login to your bank without these, however automatic they are! If you call HMRC in the morning they may be able to help, although I also believe they send out by post.
