Lta & ufpls

I took a small UFPLS from my SIPP a few months ago. Now I'm about to start receiving an occupational pension and they want to know what proportion of my LTA is represented by the UFPLS. Am I right in thinking it's just the pre-tax amount over £1m?
Supplementary question: the UFPLS is of course uncrystallised but the relevant form is headed Benefit Crystallisation Information. I'm guessing that does NOT mean I don't have to supply the info after all but I'd be grateful for confirmation.
Just thought of another question: when reporting the UFPLS is the name of the pension scheme "Hargreaves Lansdown SIPP" (say) or something else?
