c/ the layout is to basic
e/ why need date of birth for a simple freebie?

Ouch! there is one rule all ways check to see if there is a copyright info, year, date, disclamier and company then you be on the safe side other wise contain spam.

Looks genuine enough. Island is U2's record label. This website is probably under contract to gather people/fans details for future mailings (if you opt in).
