Wrong tax code

Hi all,
This is my first post here, so hello!
I wonder if anyone can offer any advice.
I am on the wrong tax code and so paying much more tax (PAYE) than I should. I have phoned HMRC twice to tell them that I am on the wrong tax code. Each time they tell me they have corrected the problem and will send me the correct code, but each time they send me the same tax code :wall:
I am currently employed by the NHS on around £26k/year. (Full disclosure: I also have a bank [i.e. Zero hours] contract with a different trust. This is taxed at the basic rate. I have not done any work for them since May, although I am still technically employed by them and so have not had a P45. All the tax from this employment is correct.)
According to the income tax calculator on here I should pay £224 tax per month, but last month they took £647!!! :eek:
When I contacted HMRC they said that according to their records I was earning an extra £21k/year which was untaxed, hence my current tax code & high payments. I told them on the phone that I was definitely not earning this money and was led to belive that this would solve the problem, but today I have had a letter from them with the same tax code as before. This letter also states that I will pay £6156 too little tax :eek: (no idea where this figure has come from. Using the calculator on this site, even if I put BR as the tax code, it says I should pay around £4K in tax.)
I am wondering whether this is related to my previous employment. Since last April (start of this tax year) I have worked for 3 trusts: Trust A, (from Oct 14) until end of April 16, on around £21k/year. Trust B, from end Apr 16 until beginning of Sep 16, on around £21k/year. I started with my current Trust (Trust C) in mid Sep 16, on £26k/year. On my P45 for Trust B it shows that I paid £0 income tax for that employment. However PAYE was being taken from my pay at the correct rate. I received a small refund (around £150) in my Sep payslip. I have no idea why the P45 shows I paid no tax. I'm wondering if this is the extra £21k that HMRC think I'm earning & not paying tax on? However, they confirmed they had a copy of this P45, so they should know I am no longer employed by this Trust and hence am no longer earning that salary. (Just to complicate things, I moved house in September so am not 100% sure where all my payslips are fro my employment with Trust B.)
Has anyone been in this sort of situation before?
Any advice? Or any tips on who to contact for advice?

The K means I "have untaxed income which is more than [my] tax free allowance and needs to be added to [my] wages.... So the additional tax can be collected"
The X means HMRC will review the tax I've paid after 5th April.0 -
