Reasons for missing credit card payments / contradicting Equifax/Experian report

Hi All,
I'm in the process of applying for a mortgage and my broker has applied for an agreement in principle from the lender and they have responded to ask for the reasons why I have missed 4 payments on a credit card in the last three years.
Firstly, I don't remember ever missing a payment.
Secondly, my Equifax report that was generated this week has given me an 'Excellent' rating and even lists each credit card I have had and doesn't show any late payments (only green boxes to suggest payments made on time).
Where do I stand here? I don't want to respond and make the situation worse by saying that 'I don't remember', could they have made a mistake or is Experian/Equifax not as detailed as I should hope?

You need to contact your card provider and raise a dispute or look at your card statements"It is prudent when shopping for something important, not to limit yourself to Pound land/Estate Agents"

Equifax is not the end of the story. Lenders can also check Experian and Call Credit - detail shown can differ.

I would be more than a little irritated if a broker did not request copies of all 3 CRA files before starting work.0
