Is there a template letter to ask creditors (DMP) to amend date of default?

Checking our credit files and all our accounts were defaulted in 2010/2011 but for whatever reason Co-op waited until 2013 to default our accounts.
I have payment history showing our payments to them as part of the DMP from 2010 onwards, so now will write to them asking them to move the date of default in line with our DMP and other accounts.
Our DMP and credit file show the accounts were moved to their recovery Dept in 2011 so was thinking that was a fair date to get it moved to.
Do we have much chance, and do template letters exist for this?
The debts are now owned by Link Financial too but they bought in 2014 well after date of default, so I was going to write to Co-op and Link, would this be wise? Or should I try going via Payplan who are managing our DMP first?

Go direct to the lender, payplan can't do anything, tell them you want the default date amending, otherwise you will involve the financial ombudsman.

Gosh, tips, I really don't know about that, we've done it because we've had to really - but definitely much easier now than at first.
It's like everyone says, we budget and account for everything,meal plan and shopping lists. Rarely buy new clothes, not that you could tell by looking at us as I tend to go Boden/Fat Face and other decent brands second handon ebay - they wear well and so need replaced less often.
It becomes habit rather quickly, gets harder to spend money.
I would totally recommend having a savings cushion, we didn't until very recently so things like the oven dying, needing a load of dental work meant we had to borrow money from parents to survive (luckily they could lend to us). I would also recommend ensuring you have a little treat monthly so you are not feeling too deprived - we had a lovely indian restaurant that let us bring our own so we could go out for dinner for about £20.
Also do anything for extra money, we do market research, surveys, sell stuff on ebay, over the years we must have made over £1k selling things/clothes/toys no longer needed at home - no place for sentimentality when you have kids to feed.
