I've had eye tests in vision express and David clulow in the last year and my child had one in Boots and we definitely weren't asked for our NHS number because I wouldn't have known what it was off the top of my head and didn't take medical cards with me. We were probably asked who our GP was (because eye tests sometimes reveal other medical problems) but if you don't have a GP just put the name of the surgery you intend to register with in that space. They won't check to see if you are actually registered there and provided you are not trying to claim any form of exemption from payment you won't be committing fraud by doing this.0

Asda/Tesco also don't ask for NHS number and offer eye tests for £15 (sometimes they do offer for £10) and then you can get a decent pair of glasses for £70.0
