As well as sharing details of your nominee, and your name and contact details, we’d like you to say, in 100 words or less, why your nominee is a champion, focusing on their achievements, their talents, their efforts to make a difference. The categories are Boy, Girl, Teacher, Parent or Lunch staff, and there is no limit to the number of nominations you can make.
The shortlisted entries will be judged by a panel of experts and the awards will be held on 10 June at the Telegraph offices*. As well as their School Champion Award, the winners will receive a prize of £500 each, and have their travel to the event paid. The winners will then be announced in the Telegraph Weekend section on 25 June.
