Cca request help please

I'm hoping somebody could just clarify a couple of things for me before I send off cca requests for my loans and credit cards.
Dh and I have been on a dmp with stepchange for 4 years, our debts are 2 credit cards, one with tesco, the other originally egg, then barclaycard now transferred to pra group. Plus a loan, originally alliance and Leicester, now santander which appears as satisfied on my credit file but isn't, and an overdraft with abbey, now santander
From what I've read overdrafts aren't covered by cca so I'm just concentrating on the loan and credit card
My questions are:
One of the credit cards was taken out before April 2007, does that need a different letter template? Both the loan and other credit card were taken out in 2008.
In regards to the loan, can santander change this back to defaulted on my credit file? I have rung them in the past about it and it hasn't been changed.
With the overdraft, is there anything I can do about the charges they put on the account even though we told them we were in financial difficulty? I tried to reclaim them a few years ago but got a letter from them saying i couldn't reclaim, I probably should have pushed further then but we had so many personal problems at the time I just dropped it.

Which template letter are you using ?

The reason your loan says satisfied is probebly because the account has been sold to a debt collector, that is the most common reason for this.

Thank you for replying Sourcrates, yes, that's the template letter that I intend to use.
As far as I'm aware, the loan hasn't been sold on, I'm still making payments to them through Step Change and received my bi annual statement from Santander this week.0 -

If it says settled, and you know it's not, then that is the usual reason why.

The loan has been marked as settled since July 2010, before we started the dmp in 2012. Maybe it's just a mistake on the file then...0 -
