Statement of Affairs bankruptcy form questions

I was going quite well working through my statement of affairs until I hit page 4 when I have to start putting down lists of all my accounts, debts and details.

Section 4 - unsecured creditors
Name of creditor (is this who the debt was originally with, or who has acquired the debt since)
Address (same as above, although I'd assume they'd want the address of whoever currently bought the debt)
Date incurred (how does this work for credit cards, is it account opening date for example, I really wouldn't even know that for a lot of them)
What was the debt for? Do I just named it as income tax, credit card, etc.

Section 5 - Bank Accounts and Credit Cards

First of all it says "include any current liability as shown in Section 4" so I assume in one of the four boxes they want me to include the number I declared in section four for each debt. But where?

As with section 4, am I naming the current debt holder (i.e. Link Outsourcing) or who the debt was originally with (MBNA/Barclaycard)?

Date obtained I assume is the account opening date. I have no idea what these are for almost all my accounts. I have lost track of everything long ago.

I assume this list is for all credit cards I have held that have debts in section 4? I hold none of them anymore. They closed 2-3 years ago when I started on a DMP.

Section 5.4 the same this list is bank accounts. It appears they want a list of all accounts I have held in the last 2 years plus any additional ones that were overdrawn (that may have been beyond the 2 years)? Again, if the debt has been sold, do I name who acquired the debt - or the original creditor?

If anyone can provide any guidance about these questions that would be amazing. I tried getting in touch with the StepChange bankruptcy team earlier but the had me on hold for 30 minutes and then they had gone home.

    The buyer, followed by the original in brackets.

    Yes, unless it was a specific purpose loan etc

    Remember this is basic info for the court, and just a staring point for the OR. As long as you have the debts listed the fine details can be sorted later if you are not sure of them. If the OR needs to know or clarify details such as previous debt owners etc, then they can ask you later.
