working for 111- help with form please

Hi
I'm looking to get a job with the 111 helpline but I can't work out if the form has to be completed on line and mailed to them, or printed off, hand written and sent?
After a lot of faffing, i managed to be able to type on the form (pdf) but it doesn't write in straight lines...i hope this makes sense.

The first time we said hello, was the first time we said goodbye. As the angels took your tiny hand and flew you to the sky-you forever left us breathless. RIP my beautiful granddaughter
