This sounds like it COULD just be a case of new accountant and management getting their wires crossed about who is responsible for what. With RTI though I am really surprised HMRC have not been on chasing. Although I assume the longer they leave it the bigger the fine!
It could even be that the accountant has been given the wrong reference.0 -

who ever is responsible for the pay role submits the rti there is no way anyone could get their wires crossed.The fines /penalties will be increasing every month , however HMRC won't be letting it ride so they can hopefully collect more money , if they haven't been paid since may I would expect a winding up order soon.
OP , if HMRC has a record of you and your employees working for this company , then there is no way that some of you have tax paid and others don't , it doesn't work that way.Vuja De - the feeling you'll be here later0 -

The employer could easily be sending RTI returns for some employees but not others.0 -
It seems clear that something has gone wrong since the new accountant started. It would seem to be a sensible idea to mention the issue to someone in authority (not the accountant - just in case). After all their tax and NI may not be being paid either. I wouldn't be surprised to hear that HMRC are on the phone this morning though.
ETA - has anyone checked to see if their pension payments are being made into their pension?0 -

It certainly can.
Depending on the tax code, some people can be paying tax and some might not (there is a tax free code). Also, low earners can have no tax deducted.
