For me the form comes up , but there are no headers to tell me what to put in what box. Any ideas ?
EDIT : Same effect in IE & Firefox , but got it to work in Chrome

After previously making my selection and told that free sample was on its way, I then got an email yesterday asking me to confirm my selection, on doing this I was informed that all products were out of stock 0
