Only way I could get this to submit was by using spaces in the DOB instead of / or - ie "12 12 1912" (not my DOB )

Similar problems with DOB but I used DDMMYYYY as one single block, that worked too.

Went to the Jaguars page (http://www.jaguars.com/uk/about-union-jax-fan-club.html) and it says to join you just register on NFL (Button at the top right of the link in original post), so I guess you just register and choose Jacksonville Jaguars as your favourite team.

there's a box just above the captcha which says join the fan club

This website is awful. The date would only go through in 01 02 2015 format for me.
