Quick question about "Prove it" letter

I'm writing a prove it letter as Lowell are chasing me for an old (about 5 years I think) debt that Im not convinced I owe, however I just have a quick question about which company I put in the "CREDITOR" bit on the template in here https://forums.moneysavingexpert.com/discussion/2607247. Is it Lowell or the water company I (supposedly) originally owed the money to? So it would read "I would point out that I have no knowledge of any such debt being owed to Lowell" or "I would point out that I have no knowledge of any such debt being owed to [water company]."?
Also, minor side rant, if I owed the water company money why didnt they contact me at the time? They knew I was moving out of that address as I told them so they should have phoned/emailed me! I haven't changed my number since then as far as I remember. So annoying! I'm tempted to send a complaint about that. I would have paid at the time had I know I owed them money (again, assuming I actually do).

Forgot to ask, should I include the credit reference paragraphs? They haven't mentioned anything about it in their letter but I don't want this affecting my credit rating if its not legitimate.0
