Sorry to bump, my form disappeared at first too but I clicked on the tab button a few times and it showed up again with error markers saying 'this field is required' which let me fill it in and submit it then.0

Okay what worked for me; refresh the page then click into one of the boxes to complete, anywhere else on the page and it vanishes again.0
