Cabot financial statement

Hi, sorry me again. I've received a letter from Cabot this am & they need me to complete a financial statement, on it they ask for information about my other creditors & debts I owe to them, its in relation to f&f offer. I have a completed budget from my DMC, it includes my total debt balance, but not whom I owe what to, or what I pay each month, would anyone recommend sending that or listing individual creditors? They also need confirmation of illness, this I can provide easily, just don't know how much info I should give them about other creditors, thanks again

The statement from your DMC would be sufficient.
