New P45 needed?

I left my previous job last year and was eventually issued with a P45.
I moved immediately to new employment
After issuing the P45 my previous employer made a further payment some months later in relation to underpayment of basic pay for several years. All deductions in relation to PAYE, NI, Pension etc were deducted from the extra payment.
Should I have been issued with a new P45 and how do I record this on self assessment?

No, once a P45 is issued no further P45s should ever be issued, your old employer should just advise you of the details of the extra payment, gross,tax,ni, etc.0
