Help Needed: Invoicing for Consultant work

Hi,
I am looking for a little advice on an legal requirements i need to consider.
I am about to start a ten week period of work as a consultant to a company. (It may turn out to be longer but initially it will be ten weeks.) I'm working at £14 an hour, and between 30/40 a week - self managed.
I have never worked as a consultant before - i have always been directly employed. I will be working for an old boss, at a new company , and this is my sole employment. I don't claim any benefits.
My questions are:
Do i need to set up as a sole trader / ltd company or can I just create invoices and bill directly as myself?
And advice re how to pay tax on earnings - and whether there are any pay thresholds to this?
