Limited company vs Self Employed

I would be very grateful for advice on whether I should opt for self assessment to go for a Limited company option. I work for the NHS as a Doctor and earn 94,000 p/a and my wife earns 50,000 p/a . We send our kids to private school and so it is a major financial commitment. To boost the income, I am planning to do private work.

Recently , I have started doing private work that will probably give me an extra £12,000-£15,000 p/a. Is it a good idea to do a self assessment or start a limited company?.
