I would be grateful if anybody could help me for a friend. He is on 61 per week carers allowance and 53 per week for working tax credits. Would he be eligible for the medical exemption certificate? He is also self employed but business running at a loss hence why top up if tax credits. Thanks for any advice.

The GP will offer your friend an application form if your friend qualifies.
But as above you have posted on the wrong board.

The medical exemption certificate is for people with certain specified medical conditions. The benefits forum will help him claim on the grounds of low-income if necessary.
