if a declare a debt from Poland in my bankruptcy application will it be written off in Poland once bankruptcy order is granted?

Yes the debt would be covered by a UK bankruptcy. I assume that you live in England or Wales and have done so for at least a year, and intend to remain here (for some time).
43.0.24 EC Regulation - automatic recognition, and the effects of that recognition
Possibly the single most useful aspect of the EC Regulation is that the bankruptcy order made in England and Wales will be automatically and immediately recognised throughout the EU URL="https://www.insolvencydirect.bis.gov.uk/freedomofinformation/technical/TechnicalManual/Ch37-48/chapter43/Chapter%2043-0/Part%203/Part%203%20notes.htm#1"][COLOR=#0066cc]note 1[/COLOR][/URL (except Denmark – see paragraph 41.7), and, with certain exceptions (see paragraph 41.36 of Chapter 41) the law applicable to the administration of the bankruptcy will be the law of England and Wales URL="https://www.insolvencydirect.bis.gov.uk/freedomofinformation/technical/TechnicalManual/Ch37-48/chapter43/Chapter%2043-0/Part%203/Part%203%20notes.htm#2"][COLOR=#0066cc]note 2[/COLOR][/URL.
In principle, therefore, the official receiver will avoid problems associated with having his/her jurisdiction recognised, and will be able to rely largely on the familiar principles of the law of England and Wales when dealing with the bankruptcy estate.
With certain exceptions (see paragraph 41.27 regarding secondary proceedings), the bankruptcy opened in England and Wales will have universal effect throughout the EU from the date of the making of the order, and it will not then be possible for a bankruptcy proceeding (or equivalent) to be opened in any other EU country.
