Mortgage appointment advice (next week)

I've arranged an appointment with Yorkshire BC for Monday & have been sent an email which slightly worried me:
Death - my pension has "death in service of 3*salary" is this valid?
Illness - how long is long term? My work sick pay is currently 1 month full pay, 2 months half. From September it would be 2 months full, 2 months half which would essentially cover 6 months hospitalised.
Would he enter this into the application or is he likely to say pay for PPI or it's classed as "sick cover n/a"?
Also, I am slightly concerned about the credit check. Looking at http://www.experian.co.uk/consumer/sample-credit-report.html
"Do you still have unpaid debt at your previous addresses?" - is this me personally? My dad still lives at that address & had/has bad debts.
This appointment will either go smoothly or be one of the strangest ever. Does the credit report list loans/debts & the dates they were issued or DOB of person? My dad has the same name & being his son I've lived with him. When he tried to renew his car insurance a few months ago the AA linked him to a crash I had a couple years ago because they only checked on name & address...they never considered DOB. Other insurance providers didn't have this issue so he just changed.
I would hope checks of this nature go off NI number/bank account or includes DOB. If they missed off DOB somewhere is this likely to be something that takes ages to resolve? If it shows a £15k loan taken out when I was 12 years old is the advisor able to click some "ignore" button and let it be processed still?
Edit: OK I'm not so concerned about the loans while in nappies now as apparently they only check the last 6 years?

No advisor will (or should) be trying to force any insurance products on you. However we do need to go through it with you, its not a nice conversation to have but I would rather discuss it at the outset and you decline it than in 5 years time when you need it and I never mentioned it.
Having it or not will not impact on your mortgage in anyway what so ever.
Death in service is valid but what happens if your employer pulls it as a benefit or you leave/made redundant etc?
I would not class long term as 2 months on a 25 year(?) loan.
Credit checks are normally name, DoB and address I think. There is the posiblity that the system could confuse you but if that happens you have the conversation with the lender and work through it.

But myself/the vendor were hoping for a quickish process. Is "working through the problems" something likely to be done there & then in the appointment? Or something that could stretch weeks?
Fingers crossed it should all go DOB & not be an issue though.

I imagine they would need an explanation and possibly evidence the debt is not yours, its unlikely to be a 10 minute phone call and job done.
But there may be no issues.
