While filling out (or filling in!) your own copy, remember to pause and marvel that US law provides not only a 'paperwork reduction act' designed to cut down on pointless bureacracy, but also a 'plain writing act' to ensure readability of the few documents that are needed once the paperwork reduction act has culled the waste. The US congress clearly does not lack a sense of humour.0
