Application passed to underwriter - advice urgently required

I know it's difficult to ascertain and no one can predetermine the outcome but very stressed currently!
AIP/DIP/valuation all done and passed as fine. Was expecting the mortgage offer this week. Found out via broker that the underwriter is querying something to do with an old address which is 12 years ago.
Credit file doesn't even have that address as a linked address and credit file is excellent with no defaults or issues.
What's the chances of this going through given I have no idea what this mysterious issue may be from years ago?!
Lender is Halifax.

Have you checked your credit file from all the agencies? Assuming it had a mortgage on it, was it redeemed after you moved out or can you think of any reason is would show up on your file after you moved out 12 years ago?0

Have one buy to let mortgage which I've had for 8 years, never missed a payment and all up to date. I can only think it's for some old utility bill or a bank thing which I genuinely have no idea what it is. There are no defaults on all 3 credit refs agencies so am confused.0

It's a tricky to know. I take it was a residential address or were you letting the address out at all after you moved out? I have some addresses showing that far back for whatever reason, it's just odd that they would ask about it from so long ago.0
